İnsülin İçin İlk Reçetemi Lizbon’da Aldım / I Got My First Prescription for Insulin in Lisbon

Lizbon’a, buradaki gündelik hayata, fazlasıyla alıştık.

11 Haziran’da çalışma ve oturma izni başvurumuzu da resmiyette tamamladık. Türkiye’de olduğu gibi kimlik kaybolunca verilen geçici kimlik dokümanı gibi bir kağıt verdiler. Bu kağıt bize gönderilecek vatandaş kartının bilgilerini içeriyor. Bu kağıtla da artık resmi olarak ülkenin sağlık sisteminden de yararlanmaya başladık.

Serviço Nacional de Saude adındaki SNS kurumuna ilk geldiğimizde e-mail ile başvurmuştuk. Çalışma ve oturma izni olmayanlar, geldikleri vize tipi uygunsa, geçici sağlık numarası alabiliyorlar. Bu sağlık numarası ile acil konular için sağlık hizmeti alınabiliyor. Biz de bu numarayı almıştık.

Çalışma ve oturma izni numaramızı da yine SNS’ye bildirerek bilgilerimizi güncellettik. Ve artık Portekiz vatandaşı gibi sağlık hizmeti almaya başladık.

Geçtiğimiz Cuma günü SNS’nin çağrı merkezini arayıp tip-1 diyabetli olduğumu, insülin ve Haşimato tiroidim için ilaç reçete ettirmek istediğimi söyledim. Bana bir SMS gönderdiler. Gitmem gereken sağlık birimi bilgilerini ilettiler. Ve ben ilgili yere gittim.

İlk işlemlerimi yapan kadın çalışana kendimi ve talebimi ilettim. İlgiyle destek oldu. 25 dakika sonrası için bir doktor randevusu ayarladı ve Eşref ile binanın 3. katında beklemeye başladık.

Ortak bir bekleme alanı var. Saatiniz gelince doktor gelip, adınızı seslenerek, sizi oradan alıyor.

Gittiğiniz doktor endokrin alanında uzman bir hekim olmadığından insülin ve ilaç ihtiyacını anlamak, reçete etmek ve yönlendirme yapma konularında destek oldu.

Yanımda Novorapid insülin, Euthyrox ve üzerimdeki sensör ve insülin pompası zaten var. Durumumu anlattım. Türkiye’den Lizbon’a gelmeden önce doktorum Enver Göncüoğlu’nun yazdığı bir bilgilendirme yazısını doktora verdim.

Doktor da tüm taleplerimi dinledi. Ve bir A4 çıktı alarak anlattı. Verdiği doküman bir yıl geçerli. Bir yıl yetecek insülin, Euthyrox yazmıştı. Kaç kutu olduğu da yazıyor bu dokümanda. Bittikçe bu dokümanla eczaneye giderek ilaçlarımı alabileceğimi söyledi. Erken biterse de yine gelmemi ve bu dokümanı düzenleyeceğini belirtti👍🫶

Aynı zamanda bir de endokrin konsültasyonu talebi açtı. Mektupla randevu bilgilerimin geleceğini belirtti. Ancak 28 Haziran 2024 yani bugün SMS geldi. Endokrin konsültasyon randevum da geldi 🙂

Dokümanda ise insülinleri ücretsiz temin edeceğimden tutun Euthyrox için ne kadar fark ödeyeceğime kadar her şey yazıyordu.

Buna gerçekten çok şaşırdım. Tek dokümanla, yeniden doktora gitmeden, zaruri iki ilacını eczaneye giderek ve sadece bu dokümanla alabiliyorum.

Türkiye’yi düşündüm. Sadece reçete yazdırmak için yaşanılan zorlukları, hekimlerimiz tarafından harcanan eforu, giden zamanları vs. Üzüldüm.

Eczaneye geldim. Kaç kutu insülin ve diğer ilaçlarda  almak istediğimi sordu eczacı kadın. 3 kutu insülin ve diğer ilaçlardan da 2şer kutu aldım. Çok cuzi bir miktar para ödedim. İnsülin ücretsiz. Euthyrox da kronik bir yaşam şekli olduğundan devlet %55 kadar ücret desteği sağlıyor. Alerji için yazılan ilaç içinde devlet desteği bulunmuyor.

En sevindiğim konu ise eczacının elinde Fiasp kutusunu görmek oldu. İnsülinler reçete edilirken etken madde yazılıyor. Ben Novorapid beklerken Fiasp ile karşılaşınca muazzam sevindim.

Fiasp ilk çıktığı zaman İngiltere’ye gitmiştik. Oradan satın almıştım. Hızlı Etkili İnsülin Humalog ve Ultra Hızlı Etkili İnsülin FIAsp başlığında o zamanki deneyimlerimi okuyabilirsiniz.

Fiasp ultra hızlı etkili bir insülin. Yapıldıktan 2,5-5 dk sonra çalışmaya başlıyor. Bu iyi bir katkı sağlıyor kan şekerini hedef aralıkta yönetmek adına.

Portekiz’in tip-1 diyabet ekosisteminde iyi olan ve olmayan yönleri var. Ancak dünkü aldığım sağlık hizmeti, kadın çalışandan kadın doktora ve kadın eczacıya kadar hepsinin muhteşem destekçi yaklaşımı buradaki iyi yönleri bana hatırlattı. Elbette bir de Fiasp’a ücretsiz ulaşmak 🙈🤗

I Got My First Prescription for Insulin in Lisbon

We have become quite accustomed to Lisbon and daily life here.

On June 11th, we officially completed our application for a work and residence permit. Just like in Turkey, they gave us a paper document similar to a temporary ID document provided when you lose your ID card. This paper contains the information of the citizen card that will be sent to us. With this paper, we have officially started to benefit from the country’s healthcare system.

When we first came to the institution called Serviço Nacional de Saúde (SNS), we applied via email. Those who do not have a work and residence permit can get a temporary health number if their visa type is appropriate. With this health number, you can receive healthcare services for urgent matters. We had also obtained this number.

We notified SNS of our work and residence permit number and updated our information. And we started receiving healthcare services like Portuguese citizens.

Last Friday, I called the SNS call center and told them that I am a type 1 diabetic and that I wanted to get insulin and medication for my Hashimoto’s thyroiditis prescribed. They sent me an SMS, providing the information of the health unit I needed to go to. And I went to the relevant place.

I introduced myself and my request to the female employee who handled my initial procedures. She was very attentive and supportive. She scheduled a doctor’s appointment for 25 minutes later, and Eşref and I started waiting on the third floor of the building.

There is a common waiting area. When your time comes, the doctor comes and calls your name to take you from there.

Since the doctor I saw was not a specialist in endocrinology, she assisted in understanding the need for insulin and medication, prescribing them, and providing guidance.

I already have Novorapid insulin, Euthyrox, and the sensor and insulin pump I am wearing. I explained my situation. I gave the doctor an information letter that my doctor Enver Göncüoğlu had written before I came to Lisbon from Turkey.

The doctor listened to all my requests. She printed out an A4 document and explained everything. The document she gave is valid for one year. She had prescribed enough insulin and Euthyrox for one year. This document also states how many boxes are included. She said that I could go to the pharmacy with this document whenever I run out and get my medications. If I run out early, she said I could come again and she would arrange this document. 👍🫶

At the same time, she also requested an endocrinology consultation. She said that my appointment information would come by mail. However, today, on June 28, 2024, I received an SMS. My endocrinology consultation appointment has also arrived 🙂

In the document, everything was written—from the fact that I would obtain the insulins for free to how much I would pay as a difference for Euthyrox.

I was really surprised by this. With a single document, without going back to the doctor, I can go to the pharmacy and get my two essential medications with just this document. If they run out early, I can return to the doctor to have the document updated.

I thought about Turkey. The difficulties experienced just to get a prescription written, the effort expended by our doctors, the time lost, etc. I felt sad.

I went to the pharmacy. The female pharmacist asked me how many boxes of insulin and other medications I wanted to take. I took three boxes of insulin and two boxes of the other medications. I paid a very small amount of money. Insulin is free. Since Euthyrox is for a chronic condition, the state provides about 55% cost support. There is no state support for the medication prescribed for allergies.

What made me happiest was seeing the pharmacist holding a box of Fiasp. When insulins are prescribed, the active ingredient is written. While I was expecting Novorapid, I was extremely happy to encounter Fiasp.

When Fiasp first came out, we had gone to England. I had purchased it from there. You can read about my experiences at that time under the heading “Fast-Acting Insulin Humalog and Ultra-Fast-Acting Insulin Fiasp.”

Fiasp is an ultra-fast-acting insulin. It starts working 2.5–5 minutes after administration. This provides a significant benefit in managing blood sugar within the target range.

There are good and bad aspects of Portugal’s type 1 diabetes ecosystem. However, the healthcare service I received yesterday—from the female employee to the female doctor and the female pharmacist—all of their wonderfully supportive approaches reminded me of the good aspects here. And of course, being able to access Fiasp for free 🙈🤗
